SUMMER 2021 BASEBALL CAMP LIABILITY AND PERMISSION FORM
Baseball Camp Overview: But God Ministries will host a Summer Baseball Camp June 22-July 15, 2021 on Tuesdays-Thursdays from 4:00pm-5:30pm. All youth ages 5-12 are encouraged to attend. Each student is required to have a signed registration form in order to participate. In compliance with the CDC’S Social Distancing guidelines, the number of registration slots has been reduced to 30 participants. Camp will be held at Jonestown’s Baseball Field. All participants will receive temperature checks before entry. Individuals displaying COVID-19 symptoms or any other illness will not be admitted. During camp, participants will learn the basics of baseball and participate in a baseball game. A light snack will be provided upon dismissal. Waiver of Liability : By Signing below, I understand that participation in But God Ministries Summer Baseball Camp may subject my child/children to harm/injury. I release the But God Ministries staff and volunteers from all liability, costs and expenses that could result from participation in But God Ministries Summer Baseball Camp events and activities. I agree to accept financial responsibility for the cost of my child/children participating in the program and any costs associated with necessary medical treatment in the event of an emergency.
Permission to Video/Photograph: But God Ministries staff and volunteers may photograph and/or videotape my child/children during But God Ministries Summer Baseball Camp from June 22-July 15, 2021. . I understand that the images may be used in print publications, online publications, presentations, websites, and social media.
