2021 SPORTS CAMP LIABILITY AND PERMISSION FORM

2021 Sports Camp Overview: But God Ministries will host a Sports Camp March 16, 2021-March 18,2021 from 1:30pm-3:30pm.  All K-5th grade students are encouraged to attend. Each student is required to have a signed registration form in order to participate. In compliance with the CDC’S Social Distancing guidelines, the number of registration slots has been reduced to 25 students. Camp will be held at Jonestown’s Baseball Field . All participants will receive temperature checks each day before entry. Individuals displaying COVID-19 symptoms or any other illness will not be admitted. Waiver of Liability : By Signing below, I understand that participation in But God Ministries 2021 Sports Camp may subject my child/children to harm/injury. I release the But God Ministries staff and volunteers from all liability, costs and expenses, that could result from participation in But God Ministries 2021 Sports Camp events and activities.  I agree to accept financial responsibility for the cost of my child/children participating in the program and any costs associated with necessary medical treatment in the event of an emergency. 

Permission to Video/Photograph: But God Ministries staff and volunteers may photograph and/or videotape my child/children during But God Ministries 2021 Sports Camp. I understand that the images may be used in print publications, online publications, presentations, websites, and social media.
